Captive Web Portal is not allowing some clients

We have an HMOL HM6 hive with around 50 APs, spread over 10 branches. The guest SSID connects via a captive web portal user acceptance page with a shared PSK.
We have found that most users are able to access the SSID, accept the CWP page and gain access.  However there are lots of users, with many in one branch, that are not able to see the CWP page, and are not able to receive an IP address.

Its seems to be only affecting the Guest VLAN SSID, and from the included image, the Clilent Auth Method only shows WPA2-psk.   While most of the successful connected clients show WPA2-PSK+CWP in this field.

Is there something obvious that I can test for.
Some users will go in and connect, while others at the same time will not be successful. Then the same users who couldn't connect, are able to at other times.
We have reduced the lease time of the DHCP server, as previously there were too many unique clients arriving during the 24hr lease that would exceed the maximum range, so now the lease expires after 2hrs. 
I have enabled the Periodic Updates of Client Database
On an effected iphone device, the SSID has been removed to start fresh, but the device still does not see the CWP page.

It seems to be related to the CWP perhaps.
